Radiation therapy has evolved tremendously over the years, and it is very critical to stay updated as we have become the pioneers at the Children Cancer Hospital in Egypt. We proudly announce the installation of our new Elekta Versa HD linear accelerators. They went live in May of 2016. The new machines are capable of delivering treatments with using Modulated Fixed and ARC Radiotherapy (IMRT, VMAT) techniques. These are considered to be the most advanced techniques which allows for more accurate dose delivery. The accuracy comes as the outcome of the changes in the Multi-Leaf Collimator (MLC) with variable MLC speed and variable dose rate.
For the first time in the Middle East MRI simulation is being used, this helps us precisely differentiate between the tumor tissues and the normal surrounding tissue. It definitely facilitate the use of Adaptive radiotherapy in which the treatment is adapted and changed according to the daily changes that may happen in the size or relative location of the tumor. Moreover, Image guided radiation therapy (IGRT) is using setup verifications (X-ray like images) or CBCT (Cone Beam CT) prior to or during a treatment to ensure positioning. In addition to this, there are other techniques that are used such as breathing techniques using the ABC, or motion sensations using the “Catalyst” device.
The Catalyst is a light scanning technology that has the ability to monitor and maintain the patient position during treatment and between fractions. We used other verification tools like “Clarity”, a 4 D sonographic device that determine the exact dimensions of the tumor during radiotherapy and halt the radiotherapy machine if a considerable deviation occurs. In such case the correction of such deviation is mandatory and accurately indicated by the Clarity.
Sentinel 4D scan application has been introduced in October 2020.
The application is synchronized to the CT in acquisition of respiratory 4DCT.
By this application, the motion of tumor with respiration is tracked and accounted.
